Azithromycin kills bacteria by inhibiting their protein synthesis. Every bacteria need protein for their living and reproduction. Azithromycin binds to the 50s subunit of bacterial ribosome and inhibits their protein synthesis. As bacterial protein production has been stopped, bacteria can not alive. Azithromycin is active against both typical and atypical bacteria.

Typical bacteria: Bacteria which contain peptidoglycan into its cell wall. Ex: E.Coli, Streptococcus pneumonia, Staphylococcus aureus etc.
Atypical bacteria: Bacteria which doesn’t contain peptidoglycan into its cell wall. Ex: Mycoplasmas, Chlamydia’s (they have cell wall but lack of peptidoglycan).

In respiratory tract infections (RTIs), both typical and atypical bacteria are involved like streptococcus pneumonia (typical), Mycoplasma pneumonia (atypical) and Chlamydia pneumonia (atypical).
In this case if patients treated with Amoxicillin + Clavulanic Acid, Ceftriaxone, Cefuroxime + Clavulanic Acid, only typical bacteria will be killed. So treatment may be failed.
